1. Acknowledgment of Risk
I understand that participation in basketball practices, games, tournaments, conditioning, training, travel, and other Lakes Basketball activities involves inherent and other risks. These risks may include, but are not limited to, falls, collisions with other participants or equipment, sprains, strains, fractures, concussions and other head injuries, illness, permanent disability, and, in rare circumstances, death.
I understand that basketball is a physical sport and that injuries may occur even when reasonable precautions and appropriate supervision are provided.
I voluntarily choose, or give permission for the participant named above to, participate in Lakes Basketball activities with knowledge of these risks.
2. Assumption of Risk
I knowingly and voluntarily assume the risks associated with participation in Lakes Basketball activities, including risks that may result from the actions of other participants, the condition or use of facilities or equipment, transportation to or from activities, or other circumstances associated with participation.
3. Medical Treatment
I authorize Lakes Basketball coaches, staff, volunteers, or designated representatives to obtain reasonable emergency medical assistance for the participant if they believe medical attention is necessary and a parent/guardian or emergency contact cannot be reached promptly.
I understand that Lakes Basketball is not responsible for medical expenses, transportation costs, or other expenses arising from an injury or illness, except to the extent such responsibility cannot legally be waived.
4. Release and Waiver
To the fullest extent permitted by applicable law, I release and agree to hold harmless Lakes Basketball and its organizers, coaches, staff, volunteers, officials, facility owners/operators, and affiliated organizations from claims arising out of or related to the participant's participation in Lakes Basketball activities, including claims for injury, illness, property damage, or loss, except to the extent such claims cannot legally be released or waived.
Nothing in this agreement is intended to waive or release any right or claim that applicable law prohibits from being waived or released.
5. Media Release and Permission
I understand that photographs, video recordings, audio recordings, and other media may be taken during Lakes Basketball activities.
I grant Lakes Basketball permission to photograph, record, reproduce, and use the participant's image, likeness, voice, and/or name in photographs, videos, social media, websites, promotional materials, team communications, newsletters, and other materials used to promote or document Lakes Basketball.
I understand that I will not receive payment or other compensation for such use.
If i decide to not grant permission for the participant to be intentionally photographed or recorded for promotional/media purposes, I will notifiy Lakes Basketball in writing or email. I also understand that Lakes Basketball cannot guarantee that the participant will never appear incidentally in group photographs or recordings.
6. Conduct and Safety
I agree that the participant will follow reasonable instructions from coaches, staff, officials, and event personnel and will follow applicable facility and program safety rules.
I understand that Lakes Basketball may remove a participant from an activity when reasonably necessary for safety, conduct, or other legitimate program reasons.
7. Electronic Signature and Agreement
By signing electronically below, I confirm that:
I have read and understand this waiver and release.
I understand the risks associated with basketball participation.
I voluntarily agree to the terms of this document.
The information I have provided is accurate.
If signing as a parent or legal guardian, I have authority to provide consent on behalf of the participant.
I understand that an electronic signature is intended to have the same effect as a handwritten signature to the extent permitted by applicable law.
